This is an analysis of public treatment listings, compiled 19 Sep 2026. We have not checked every listing, and a listing that names Medicaid does not mean a program takes every Medicaid plan. How we did it, and what it cannot tell you, is at the end. The city by city table is free to download and reuse: detox-deserts-medicaid.csv.
What we found
- 726 of 1,610 cities (45%) have a detox in the city itself that lists Medicaid.
- 300 cities (19%) are more than 25 miles from the nearest one in their state.
- 116 cities (7%) are more than 50 miles away, and 27 are more than 100.
- South Dakota has one program in our directory that lists both detox and Medicaid. From Rapid City, that is about 329 miles away.
- State lines matter. From Moorhead, Minnesota, the nearest Minnesota detox that lists Medicaid is about 149 miles away. Across the river in Fargo, ND, one is about 4 miles away. Medicaid is run by each state, and it generally pays for care in another state only in limited cases.
Why the distance matters
Detox is often the first step, and it is the one that is hardest to wait for. Withdrawal from alcohol, benzodiazepines or opioids can start within hours, and alcohol withdrawal can become a medical emergency, as benzodiazepine withdrawal can too. A family that finds a program two hours away has to arrange transport for someone who may be sick, at the moment they have agreed to go. When the nearest option is out of state, the question of whether Medicaid will pay adds another call, and often another delay.
If someone may be in withdrawal and is confused, having a seizure or hallucinating, that is an emergency. Call 911.

Where a state line hides a closer one
In these cities, a detox that lists Medicaid is much closer across a state line than the nearest one in the same state. Whether a family's Medicaid will pay for it depends on the state and the circumstances, so the program and the state Medicaid office are the ones to ask.

If you are looking for a detox now
- Start with the programs in your state that list Medicaid: programs that list Medicaid, by state. Every city page shows the levels of care nobody there lists, and the nearest city that does.
- Call before you travel. Ask whether they take your Medicaid plan, whether they have a bed, and what happens when detox ends. Our guide to verifying coverage before admission has the questions.

How we did it
- The listings. 17,236 addiction and mental health treatment programs from public treatment directory listings, compiled 19 Sep 2026 and filled in from SAMHSA's FindTreatment.gov where they matched. We have not checked most of them. See how listings work.
- A detox that lists Medicaid is a listing that names detox, also called withdrawal management, as a level of care and names Medicaid as coverage: 1,747 programs. A hospital that manages withdrawal without listing detox is not counted, so some places are closer to care than this shows.
- The cities are the 1,610 cities with at least 3 programs in our directory, in the 50 states, Washington, DC and Puerto Rico. Towns with fewer programs are not in the count, and they are more likely to be far from a detox, so this probably understates the gaps.
- Distances are straight lines between the middle of a city's listings and the nearest detox. Program locations are the centre of each program's ZIP code. Driving distances are longer.
- Same state. Medicaid is run by each state, and federal rules let a state pay for care in another state only in limited cases (42 CFR 431.52), so the main measure is the nearest detox in the same state. The nearest across any state line is in the download too.
- Lists Medicaid means a program's public listing says it takes Medicaid. It does not mean it takes every Medicaid plan, has openings, or takes new patients.
The download has one row per city: the city, the state, how many programs we list there, how many of them list both detox and Medicaid, the nearest in the same state and its distance, and the nearest in any state and its distance. It is free to reuse with a credit to findingrehab.io.
